What to expect on your first visit to our office

For your first visit please:

• If you have insurance, please have a current insurance card with you.

• A Photo ID (Driver's License, State Issued ID, Passport, Federal ID)

• Your first visit to our office must be paid by you.  (*Please note, we are a Non-PPO office)

• We will gladly bill your insurance for you.
